Understanding Lactose Intolerance vs. Dairy Allergy

What Is Lactose Intolerance?

Without sufficient lactase, lactose passes into the large intestine, where it is fermented by bacteria, leading to digestive symptoms.
Common symptoms include bloating, abdominal cramping, excessive gas, diarrhea, nausea, and stomach discomfort that typically develop within a few hours of consuming dairy products. The severity of symptoms often depends on how much lactose is consumed and the individual's ability to digest it.

What Is a Dairy Allergy?

A dairy allergy, also known as a milk allergy, is an immune system reaction to proteins found in milk rather than an inability to digest lactose. When someone with a dairy allergy consumes milk or dairy products, the immune system mistakenly identifies the proteins as harmful and triggers an allergic response.
Symptoms may include hives, itching, swelling of the lips or throat, wheezing, coughing, vomiting, or digestive upset. In severe cases, a dairy allergy can lead to anaphylaxis, a life-threatening allergic reaction that requires immediate emergency medical attention.

Key Differences Between the Two Conditions

Although both conditions may cause digestive symptoms, lactose intolerance affects the digestive system, while a dairy allergy involves the immune system. Lactose intolerance is generally uncomfortable but is not life-threatening. A dairy allergy, however, can range from mild to severe and may require strict avoidance of all milk proteins.
People with lactose intolerance can often tolerate small amounts of dairy or lactose-free products, while individuals with a dairy allergy typically need to avoid dairy products entirely unless otherwise advised by their healthcare provider.

How Are These Conditions Diagnosed?

A healthcare provider will review your symptoms, medical history, and the timing of your reactions to dairy products. Lactose intolerance may be evaluated using dietary elimination, hydrogen breath testing, or other diagnostic methods. A suspected dairy allergy may require allergy testing or referral to an allergy specialist.
An accurate diagnosis is important because treatment recommendations differ significantly between the two conditions.

Managing Symptoms and Protecting Digestive Health

The best management approach depends on the underlying diagnosis. People with lactose intolerance often benefit from limiting high-lactose foods, choosing lactose-free dairy products, or using lactase enzyme supplements when appropriate. Those with a dairy allergy should carefully read food labels and avoid products containing milk proteins to prevent allergic reactions.
